A consultation on the Strategic Plan for integrated health and social care in Clackmannanshire and Stirling has been launched.
The Clackmannanshire and Stirling Health and Social Care Partnership wants as many views as possible on a draft Strategic Plan, which outlines how the NHS and Councils in Clackmannanshire and Stirling will work together to provide services over the next three years. The Plan was developed using feedback from the public, staff, people who use health and social care services, unpaid carers, community groups and providers.
The integration of Health and Social Care services is aimed at delivering more personalised, joined-up care, especially for people with long-term conditions and disabilities, many of whom are older. Similar reforms are happening in Councils and Health Boards throughout Scotland.
Chief Officer for new Clackmannanshire and Stirling Health and Social Care Partnership Shiona Strachan said:
“We want to hear your thoughts on the draft strategic plan. What matters to you is important to us, and this is your opportunity to influence the way local health and social care services are developed and delivered.”
